You can save considerable time during your first visit by printing the appropriate form (listed below) for your child, completing it at home, and bringing the completed form with you to your first visit. The patient, family, and social history is an essential part of the medical record and will be transferred to your child’s secure, permanent electronic health record. 

The PATIENT INFORMATION FORM (Adobe Acrobat format) should also be completed prior to your first visit with us or if you are an existing patient and need to update your information on file with us.
